Why do patients with ichthyosis develop "elderly-looking" hands and feet?

 

In patients with ichthyosis, a genetic defect impairs the skin's metabolic function. Consequently, dead skin cells—which have perished prematurely—adhere tightly to the skin's surface instead of shedding naturally. This leads to an abnormal thickening of the stratum corneum (the outermost layer of skin) on the palms and soles, resulting in a rough, dry, and cracked texture.


Furthermore, because the skin of ichthyosis patients is unable to effectively retain moisture—and given that the hands and feet possess the fewest sebaceous glands—this extreme lack of hydration causes the skin to develop fissures and cracks of varying depths.


Ultimately, this causes the hands and feet to resemble those of an elderly person or someone engaged in prolonged manual labor—characterized by worn-down skin, thickened calluses, deep palm lines, and cracked heels.


In summary, this condition is the result of the combined effects of localized hyperkeratosis (excessive thickening of the skin), severe dryness, and a deficiency in sebum production; it does not indicate actual premature aging of the body, as even some children suffering from ichthyosis can exhibit these "elderly-looking" hands and feet.

The condition can be improved through effective treatment to remove the surface scales from the hands and feet, combined with the use of specialized skincare products designed for ichthyosis.
